Grandfamilies Survey of All Utah Relative Caregivers 2016-2021
This survey should take about 20-25 minutes to complete and is the first of two surveys we will be asking you to take. In this survey we want to ask you a few questions about your current and past caregiving experiences as caregivers to a relative or friend’s child, so that perhaps we could better provide support to you and other caregivers in similar circumstances. The results of this survey will provide information that will be used to help agencies within the state of Utah to better support you. We ask that no one under the age of 18 take this survey.
In this survey, you will be asked to share your opinions and understandings. In addition, you will be asked to provide some information about yourself and the child or children you provide care to. For this study to be successful, we have to ensure that your anonymity and confidentiality is absolutely protected. We hope that you can trust that it will be. We want you to know that the circumstances surrounding your experiences are sensitive and extremely private. We will keep all your answers strictly confidential. We never ask you to put your name on the survey and will also keep all your answers safe and confidential. This means that all of your personal answers such as county of residence or IP address will be removed and your individual responses will not be shared with anyone outside of the research group and the supervising professor. Only aggregated information will be made available to others.
This scientific study is being supervised by Dr. Ron J. Hammond, UVU Professor of Sociology (801-863-8344, firstname.lastname@example.org ). Dr. Hammond volunteers his research expertise in support of you the caregiver, the children, and the agencies that provide support to you and others in similar circumstances. Your participation in this study is completely voluntary. You may choose not to participate, you may choose not to respond to particular questions, or you may also discontinue participating at any time without penalty.
